Job Description/Requirements
Responsibilities:
- Accurately prepare and dispense medications.
- Review and interpret prescriptions to ensure safety and appropriateness.
- Provide drug information and counseling to patients and healthcare professionals.
- Monitor patient drug therapies and intervene as necessary.
- Conduct medication reconciliation for patients during admission, transfer, and discharge.

- Stay updated on new drug therapies and best practices in pharmacy.
Requirements:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Minimum of 2 years of experience, preferably in a hospital setting.
- Strong knowledge of pharmacology and medication management.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively.
- Detail-oriented with strong problem-solving abilities.
